ABSTRACT

A total of one hundred and thirty random milk and some milk products samples were collected from different markets, and were examined for the presence of Escherichia coli O157:H7. The organism could be isolated from one sample [2%] of each of milk and cheese samples, but it failed to detect in yoghurt samples. An important cause of haemolytic anaemia syndrome and lethal effect on mice was detected, while enterotoxin of strain gave maximum secretion in rabbit ligated ileal loop Raw milk and milk products samples [cheese and yoghourt] free from E. coli O157:H7 were inoculated by strain of E. coli O157:H7 obtained from a Bacteriology Veterinary Lab. The effects of irradiation on the inoculated milk and milk products stored at 8°C -and 5°C were determined. As survivor level of E. coli O157:H7 following gamma irradiation appeared completely discarded in refrigerator on 1, 2 and 3 days of incubation at 5°C. The cytotoxin production was completely stopped by irradiation at 0.25-0.53 kgy

ABSTRACT

With the evolution in surgical technique, anaesthetic care and postoperative management, major hepatic resections for giant liver tumors are now considered reasonably safe procedures with a low surgical death rate. The aim of this study is to evaluate the results of major hepatic resection for giant hepatic tumors in the National Liver Institute during the period between September 1991 and September 2001 and to compare these results with reports from other centers. From September 1991 to September 2001 twenty-six giant liver tumors undenvent major hepatic resection out of a total number of one hundred and thirty seven liver tumors that were resected in the National Liver Institute. The tumor diameter was ranging from 8 cm to 27 cm. There were 6 children: their ages ranged from 8 months to 13 years. Four of them had hepatoblastoma, one malignant mesenchymoma [undifferentiated embryonal sarcoma] and one case of liver cell adenoma. The adult group included 20 cases with a mean age of 52.5 years [range 26-65 years]. There were six cases of hepatocellular carcinoma [HCC]; two of them were done as an emergency due to bleeding after tumor rupture. The remaining 14 cases were: 2 cases of HCC fibrolammellar type, one gallbladder carcinoma, 2 cases of colorectal liver metastasis, 5 cases of haemangioma, one case of haemangioendothelioma, one case of secondary leomyosarcoma from the stomach, one case of cholngiocarcinoma and one case adenocarcinoma. Right or extended right hepatectomy was done in 14 patients and left or extended left hepatectomy was done in 12 cases. Ultrasonic dissector and bipolar coagulation was used in 21 cases. Hospital mortality was three cases; two of them were emergency cases on top of chronic liver disease. Postoperative complications included: temporary ascites, chest infection, minor biliary leak and wound infection. major liver resection is a reasonably safe procedure especially, when performed on normal liver under elective conditions

ABSTRACT

1- The use of proteolytic enzymes trypsin and chemotrypsin enhances the healing process of jaw fracture and significantly reduced the degree or both post-traumatic and post-operative edema when used in different cases of fractures of the facial skeleton. 2- To achieve satisfactory healing of cysts, the authors recommend treatment by enucleation, together with reduction of the size of cyst cavity by cutting away expanded cortical plate of bone and putting in mattress surfaces. At the same time, the patient should be given prophylactic antibiotic therapy and proteolytic enzymes. 3- Trismus and pain suffered by a patient who has undergone surgery for removal of lower third molars can be considerable reduced by the use of proteolytic enzyme therapy. 4- The types of bacteria giving rise to odontogenic or non odontogenic cervicofacial abscesses have been defined in this study into, streptococci and staphylococci. Streptococcal infection is best treated by ampicillin or erythromycin. For staphylococcal and gram negative infection cotrimoxazole, erythromycin or a cephalosporin, where no differentiation is possible or mixed bacteria is present erythromycin is the first choice. Accordingly the proper selection of antibiotic, proper drainage together with proteolytic enzyme therapy which was found to be effective in the rapid debridement of the abscesses, is our recommended line of treatment of all cervicofacial abscess

ABSTRACT

The present study was undertaken to determine the effect of halothane, nitrous oxide and surgery on plasma thyroid-stimulating hormone [T.S.H.] in 20 patients by utilising a radio-immuno assay technique. Measurements of serum thyroid levels were performed simultaneously. The study demonstrated that neither halothane, nitrous oxide anaesthesia, nor surgery influenced appreciably the plasma T.S.H. concentration. However, serum thyroxine level in the halothane group increased significantly, though no significant variation in serum thyroxine level was detected in the nitrous oxide group. These data would suggest that T.S.H. appears to play little role in the control of serum thyroxine level under the stress of anaesthesia and surgery in man

ABSTRACT

The effects of changes in the acid base balance and electrolytes on the degree of neuromuscular blockade were studied on 60 adult patients undergoing gastrointestinal surgery. The cases included 30 normal patients and 30 with late intestinal obstruction. For the relaxation during surgery, three types of non-depolarizing muscle relaxants [d-turbocurarine, flaxedil and pancuronium] were used. A peripheral nerve stimulator was used to assess the degree of neuromuscular block. The study showed that metabolic acidosis and hypokalemia let to prolongation of the peripheral effects of three types of non-depolarizing muscle relaxants used with varying degree of affection. This was evidenced by decreased latency, increased time to return to the pre-relaxant response after reversal, and marked prolongation in the recovery time. The study showed that pancuronium is the drug of choice in late cases of intestinal obstruction, being easily reversed and not greatly affected by electrolytes and acid-base disturbances

ABSTRACT

The polylactic acid surgical dressing material was used in fifty patients with bilateral soft tissue impaction, the right side as control group and the left side as study group. The results of this study showed that the polylactic acid has a significant ability to reduce the pain. On the other hand, the stability and the healthy condition of the blood clot was significantly obvious in the study group. Therefore the polylactic acid was significantly effective in the prevention of dry socket particularly in mandibular third molar wounds. Also in this study the experimental work was done in albino rat to show if the effect of this material on the sequence of healing of extraction wound and also to study any tendency for dry socket. The histological findings in the present work revealed that PLA dressing material significantly reduces the incidence of dry socket by supporting the wound's granulation tissue with available blood supply and in contact with that enhances new bone formation. Also this investigation showed that the rate of degradation PLA is very slow, but without any evidence of inflammation in one month study. So this can help very much in plastic surgery

ABSTRACT

This work was carried on thirty six male albino rats. Extraction of upper right incisor was done for each animal. The animals were divided into three groups. Histological study of the healing of the extraction socket filled with aspirine and Alexoprine were compared with control group. This study revealed that healing of Alexoprine group was similar to the control group while there was a delay in the healing of aspirine group especially in the second and third week when compared with the control group
